This form is to give authorization for another homeowner to cast your vote, if applicable, at a HOA meeting. (Please Note: Proxy forms are not applicable for anyone wishing to become a Board Member...you must be present to obtain a Board Seat).

I (we, if jointly owned), being the person(s) whose name is/are identified as the Owner(s) of the Unit listed in this form, as evidenced by submission to, and confirmation from, the Property Manager, do hereby constitute and appoint by Proxy Holder, also identified on this form, to attend the meeting identified here, with full power to vote and act for me/us in my/our name, place and stead, in the same manner, to the same extent, and with the same effect that I might vote and act, were I personally present at such meeting, and I hereby revoke any other proxy heretofore given by me.
